Instructions:

 

1. Click on Membership Application

2. If you have any questions, please call (808) 587-2700 and a Member Service Representative will be happy to assist you.

 

3. Upon completing the application, print and sign the form, then mail to:

4. We also require you to send with your completed and signed application a visually clear photocopy of a valid form of photo identification, with the exception of a birth certificate for minors. The following are the only acceptable forms of identification.

  • Driver’s License
  • State of Hawaii Identification
  • Military Identification
  • U.S. Passport
  • For minors without a valid form of identification, please include a copy of their birth certificate.
